Johnson Rd near Middletown Rd

Smith Twp PD recieved a call that a white van on 
Johnson Rd near Middletown Rd was off the 
roadway with its four ways on. When I arrived in 
the area and found the van (FAP4783). I spoke 
with Charles Ketler who advised he hit a large pot 
hole then his vehicle had shut off. 

Mr. Ketler tried to start his vehicle again and it 
failed to start. Mr. Ketler requested K&D Towing to 
come get the vehicle. Once the
vehicle was removed I cleared the scene.

Animal Complaint
13897 Thompson Street

This oficer was dispatched and responded to 
13897 Thompson Street in regards to an animal 
complaint. Upon arrival contact was made with 
Mr. Richard Percy and his wife Mrs. Dorothy 
Percy. The subjects advised that the neighbors 
dogs were continually loose and they were fed up 
with it. The subjects also confronted and spoke to 
the neighbor as he was attempting to leave while 
on scene identified as Mr. Jordan L. Riggs. Mr. 
Riggs stated that the Percy's also have animals 
that are always on his property as well. After 
speaking with both subjects and advising them to 
keep their animals tied up, the scene was cleared.

UNIT#1 was traveling South on Beloit Snodes Rd 
approx 50 Ft North of 11190 Beloit Snodes. Unit#l 
lost control and traveled off the right side of the 
roadway striking a ditch. Unit #1 then traveled 
138.10 Ft along the ditch until it struck a Century 
Link phone pole. 
Unit #1 then struck a fence belonging to 11190 
Beloit Snodes Rd, and then traveled another 40 ft 
for a total of 178.10 ft till coming to rest in the 
ditch.

Mr. Erath who was also the registered owner of 
this vehicle. He stated that he forgot that his 
plates were expired, the driver was cited under 
4503.10 ORC. The vehicle was towed by 
Springers. The drivers insurance was expired as 
well.

CITED: For Speed and for possession of 
Marijuana

While sitting stationary on Todd at Lake Park, the 
defendants vehicle, a red Chevy pickup was 
observed traveling west bound. The vehicle 
appeared to be traveling faster than the posted 40 
mph speed limit. The hand held radar was 
activated, a strong audible tone was heard, and 
the target speed reading of 57 mph was obtained. 
A traffic stop was then initiated. Before 
approaching the vehicle Officer noticed both 
subjects moving very rapid and frantic. 

Once Officer made contact the defendant was 
identified as Scott W. Womer Jr. 

While explaining the reason to Mr. Womer why 
Officer stopped him Officer noticed a very strong 
odor of burnt Marijuana. Mr. Womer immediately 
became very defensive and advised he was not 
speeding and demanded to come back to my 
cruiser (car #31) and look at my radar unit. Officer 
advised Mr. Womer for my safety of him getting 
out of the vehicle and him walking along the 
roadway Officer would not take him to my cruiser. 

I then questioned Mr. Womer about the smell of 
Marijuana. Mr. Womer again got very defensive 
and changed the subject several times going back 
to claiming he was not speeding. At that time 
Officer asked Mr. Womer if he had Marijuana or 
anything else in the vehicle that he should not. 
When Mr. Womer would not cooperate Officer 
requested mutual aid from Alliance PD to see if a 
K9 unit was available. 

Mr. Womer quickly changed his attitude and 
handed me a bag with a small amount of 
Marijuana. Mr. Womer then started to claim that it 
is not his and that it belongs to a friend he is 
employed with.

Mr. Womer stated the smell is from letting other 
employees whom he works with at VacuForm 
smoke Marijuana in his vehicle in the parking lot 
of his employment. Mr. Womer once again tried 
changing the subject stating he was only doing 50 
mph in a 40 mph zone and that my radar unit 
needed calibrated. 

Mr. Womer was cited under ORC 4511.21 for 
Speed and ORC 2925.11 for possession of 
Marijuana with a court date of 1/22/15 at 11 :00 
hrs at MCC# 3. The bag containing Marijuana was 
placed into the evidence locker.

per request of Alliance PD to help locate a male 
subject that was involved in a hit/skip accident in 
their jurisdiction. Dispatch advised that APD 
already had a unit en route as well. Upon my 
arrival APD officers were already on scene and 
had made contact with the subject. This officer 
stood by to make sure everything was under 
control. This officer then cleared.

On this date and time Officer responded to 13990 
Oyster Rd for an unwanted subject. When Officer 
arrived Officer spoke with Monetha Jarvis. Ms. 
Jarvis stated her son In-law, Jason Rhea, would 
not leave the property when she asked him to. 
Ms. Jarvis stated everything is alright now and Mr. 
Rhea had left.

At the time Officer went next door to 13996 Oyster 
Rd to make contact with Mr. Rhea. Mr. Rhea 
stated he went next door to retrieve some items 
that belong to him when Ms. Jarvis began to get 
vulgar with him. Mr. Rhea advised he had the 
items he needed and had no need to go back to 
her residence. At that time Officer cleared with no 
incident.

THEFT-FROM AUTO

On 01-16-2015 at approximately 0446hrs Officer 
was dispatched to 22734 Alden Ave in reference 
to a possibly theft from vehicle. When Officer 
arrived on scene Officer made contact with Edna 
who was the caller and victim. Edna stated that at 
approximately 0430hrs she went to start her 
vehicle to warm it up and returned inside. 
Approximately ten to fifteen minutes later she 
returned to the vehicle and noticed the driver side 
front door was ajar. At this time she looked inside 
the vehicle and noticed that someone had 
rummaged through the vehicle. 

She stated that she did not see anything missing 
from her vehicle.

Car vs. Deer

On Friday, January 16,2015 this officer was 
dispatched and responded to the 22000 block of 
Courtney Rd for a car vs. deer accident. 
Ptl. Baker and myself responded to the scene. 
Upon arrival Officer had spoke to the driver and 
registered owner of the vehicle Ms. Celeste K. 
Gallagher that stated the deer had jumped out in 
front of her vehicle causing her to hit it. Subject 
stated that she was not injured and did not need 
medical attention. 

Ms. Gallagher requested that an accident report 
not be done because she would not need it for her 
insurance company ( Nation wide) and that she 
did not want it added to her record. The deer was 
observed in the middle of the road that appeared 
to be injured. 

The deer was put down by Ptl. Baker. 4 shots 
were fired. Once the animal was deceased the 
animal was released to Ms. Gallagher with an 
animal release form.

Friday, January 16,2015

On Friday, January 16,201 5 Ms. Jennifer L. Floor 
had came to this department stating that she was 
advised by her attorney to come to this 
department in reference to her husband that had 
purposely destroyed certain parts of their house 
leaving it a complete mess making it seem that 
she was living in unfit living conditions not suitable 
for her children and he had also took pictures of 
the mess that he had made and sent them to 
Children protective services. 

Jennifer advised that the house was not like this 
prior to him going in and that the house is never 
left in unfit and poor living conditions. Jennifer 
stated that her daughter was able to verify by 
looking at the pictures that the house has never 
looked the way the pictures present and found the 
pictures to be very disturbing. A copy of the report 
will be requested from CPS and this case will be 
forwarded over to Detective Starr.

On 01/1 8/2015 at approx 1809hrs Officer was 
dispatched to the 22000 block of Lake Park Blvd 
for a suspicious male who was walking in the 
middle of the street. Upon my arrival Officer made 
contact with the male (Christian Carver).

Mr. Carver advised me he was walking home from 
his cousins house who lives on Summit St. Officer 
advised Mr. Carver he can not be walking in the 
middle of the roadway and advised him of the 
hazards he could face. He said he was trying to 
get home to 623 W Oregon St in Sebring. Officer 
advised him Officer would give him a courtesy 
transport to his residence. Officer then gave him a 
transport to his residence without incident.

(Eddy Andrew). Mr. Andrew stated that the 
garbage company comes tomorrow and will pick 
up everything. I then cleared from the scene.

are pushing the car towards Damascus on 62.

SUSPICIOUS VEHICLE

On 01/04/2015 at approx 21 13hrs I was 
dispatched to 17394 U.S 62, just one house east 
of that address for a suspicious vehicle. The caller 
advised dispatch that the vehicle had driven 
through her neighbors yard and their steps, and 
that her neighbors are not home at the time. While 
driving to the call dispatch stated that the males 
are pushing the car towards Damascus on 62. 

Upon my arrival I patrolled the area and did not 
see any vehicles matching that description. I 
advised dispatch to let Goshen PD know of this 
incident. I went to the residence where the 
incident occurred and did not observe any 
damage to the property. The residents were not 
home at that time.

On 111 112015 at approx 1520 hrs I went to the 
victims house ofwhere the incident took place to 
follow up.

Upon my arrival I noticed no damage to any steps 
nor no damage to the lawn. I tried to make contact 
with the homeowner but no one was home. I also 
tried to make contact with the homeowner 
throughout the night but no one would answer the 
door.

I then made contact with the witness residing at 
17394 ST RT 62 (Tamre Wilson) Ms. Wilson 
stated that last Sunday she saw three males 
trying to push a white or silver PT Crusier out of 
the driveway and then noticed that they were 
trying to get it on a trailer that was parked in the 
driveway of 17396 ST RT 62. She stated that 
what she heard was loud noise and thought it was 
the vehicle hitting the steps but then realized it 
was the
males trying to load up the vehicle on the trailer. 
She stated that the males finally loaded up the 
vehicle on the trailer and drove away, she was 
unable to give me a plate or a vehicle description 
of the vehicle pulling the trailer.

Police were called to 1005 Lake Park Blvd. Lot 83 
for a menacing by stalking report.

On 01-03-2014 at 17:55 hours officer was 
dispatched and responded to 1005 Lake Park 
Blvd. Lot 83 in reference to a report of menacing 
by stalking. Upon my arrival officer spoke with 
Melinda Hall who is staying with a friend that lives 
at Lot 83. Ms. Hall stated that she and her 
husband haven't been together for approximately 
a year.

She moved to Ohio from Florida to get away from 
he husbandex husband Larry due to unfortunate 
circumstances.

Ms. Hall stated that Lany was at her friend's 
house on the south side of SR 62 trying to see if 
Melinda was there. Melinda stated that her fiiend 
was afraid to leave her own home because Larry 
was sitting in his car outside her residence. officer 
advised Ms. Hall that if her friend is concerned 
then she needs to contact Columbiana County SO 
and make a report with them since she is in their 
jurisdiction.

Ms. Hall stated that she doesn't want anything to 
do with Larry and is trying to stay away from him. 
She also advised that Larry doesn't have her 
phone number anymore and has not had any 
contact. officer advised Ms. Hall that in order to 
make sure Larry leaves her alone she needed to 
go to Youngstown and file for a restraining order 
for her own safety.

The matter concerning Melinda's friend was 
referred to Columbiana County SO.

On 01/10/2015 at approx. 11:31 hrs. while on 
patrol near the 22400 block of SR 62 officer 
observed the defendant, Zachary D. Zufall, who 
officer knew at the time to be suspended. At that 
time officer performed a LEADS check to which 
Mr. Zufall was suspended. A traffic stop was 
initiated and contact was made with Mr. Zufall. Mr. 
Zufall stated he paid the court to receive his 
license back. officer advised Mr. Zufall that it still 
shows in the LEADS system that he is 
suspended. officer also advised Mr. Zufall that if 
he can find a valid driver in a reasonable amount 
of time they can drive the vehicle. Once Mr. Zufall 
had someone on scene to drive his vehicle, Mr. 
Zufall and the vehicle were released. Mr. Zufall 
was cited under ORC 45 10.1 1 Driving Under

This unit arrested Andrew Nutt on a warrant. 
officer contacted Columbiana Police department 
and requested they check Mr. Nutt's residence. 

They called back and advised they had him in 
custody. officer responded to their station and 
took custody of this subject. officer transported 
him to the Mahoning County Court # 3 to appear 
in front of the Judge. He was then turned over to 
the Mahoning County Deputy Mitchell for 
transported to the Mahoning County jail.

Accident - Property Damage Only

CAR IN DITCH ON 12TH ACROSS FROM 
ALABAMA

On 01/14/2015 at approx. 07:01 hrs. this officer 
was dispatched and responded to the area of 12th 
street and Alabama Avenue in regards to a 
vehicle in a ditch. Upon arrival the vehicle was 
located and found to have no damage. Contact 
was made with the driver identified as Mr. James 
D. Bevins 11. The subject advised that his 
windows were covered in ice and he was unable 
to locate the roadway. Mr. Bevins instead traveled 
down an old driveway before becoming stuck in 
the yard. Contact was made with the homeowner 
who advised that he
was not concerned about his yard. Mr. Bevins was 
able to remove his vehicle and the scene was 
cleared.

On Monday, January 12,201 5, Officers 
responded to the wooded area of Railroad St near 
Stanley Ave. to assist APD units who were in a 
foot pursuit with Joshua Percy who was wanted 
on felony charges.

Officers assisted with the foot pursuit until being 
asked to standby at the suspect's residence at 
1520 Norman Ave. Officers stoodby at the 
residence until1 dispatch advised that APD untis 
had captured the suspect at the train station. 
Officers then cleared.

On Monday, January 12,201 5 this oficer was 
dispatched and responded to 995 W. Ohio for a 
property damage complaint. Upon arrival officer 
spoke with Ms. Darci L. Davis. Ms. Davis advised 
that sometime in the early morning hours on this 
date that a plow truck had came through and hit 
her mail box causing damage.

Subject stated that she wanted it fixed. officer had 
advised this subject that she would need to 
contact either the Township garage or county to 
advise them of the incident. Once the scene was 
secure this officer then cleared.

WARRANT

CHARLES W CALDWELL - 1005 LAKE PARK 
BLVD SEBRING

On 01-07-2015 at approximately 0013hrs I 
observed that Charles Caldwell's vehicle was park 
at his place of employment (Whitacre Greer). At 
this time I requested for an additional officer to 
assist with finding the location of this person of 
interest. When Sgt. Faudre arrived on scene we 
were greeted by the building foreman. The 
foreman located Mr. Caldwell and he was then 
placed into custody by me. 
At this time I advised the subject of his warrants 
and then I read him his Miranda rights. He was 
then transported to Smith Twp Police Department.

After completing the proper paperwork the subject 
was then transported to Mahoning County Jail.
